Report of Death of A Filipino Abroad

ONLY deaths of Filipinos in the following places can be registered or reported at the Philippine Consulate General, Los Angeles, CA: (1) Southern California; (2)Southern Nevada (Clark, Lincoln and Nye Counties); and (3) Arizona.

Procedures and Requirements:

Send to the Consulate BY MAIL the following documents:

1. Four (4) original duly accomplished NEW Report of Death Form (either type written or legibly handwritten) should be executed by next of kin or the attending physician or nurse or their duly designated representative. The Report of Death form must be notarized with the notary public signing AND acknowledging that the one reporting the death personally appeared before the notary public.(click here for the fillable form)

For death prior to the complete expulsion or extraction from its mother of a product of conception, irrespective of the duration of pregnancy click here for the NEW FETAL DEATH FORM.

2. The original copy of the Certificate of Death issued by the local health authorities or by the local County Registrar must be presented. The Certificate of Death should also be reproduced in quadruplicate (four copies).

3. Valid Philippine passport of the deceased must be submitted, also reproduced in quadruplicate (four copies), in order to determine Filipino citizenship. In the absence of the valid passport, any other form of identification showing Filipino citizenship such as (a)  Dual Citizenship (RA 9225) Documents, (b) NSO/PSA Birth Certificate [you can apply for the birth certificate here ] with NOTARIZED affidavit of next of kin certifying that the deceased has not acquired foreign citizenship or (c) expired Philippine passport with NOTARIZED affidavit of next of kin certifying that the deceased has not acquired foreign citizenship.

4. The fee for this service is $25.00 in cash, postal money order or cashier’s check payable to the Philippine Consulate General. Personal checks, peronal money orders, ATM and credit cards are not accepted as mode of payments. Do not send payment in cash when reporting by mail.

5. Include in the package a USPS Priority Mail Flat Rate Envelope (or Box if you have really thick documents)  that is self-addressed and postage prepaid as the return envelope with USPS Tracking To compute the amount of postage for a USPS Flat Rate Envelope, check here.

Please take note of the mailing or courier tracking number since the Philippine Consulate General cannot assume responsibility for documents which have been mailed through the U.S. Postal Service or courier service.

From May 15, 2019, the Consulate cannot authenticate the signatures on the following public documents:(1) notarized documents and (2) documents signed/issued/certified by a Federal, State, County, City, University or School Official.

The change is due to the Philippines officially becoming a party to the Apostille Convention.
