Reporting Births of Children of Filipino Citizens
The Report of Birth is a declaration of the person furnishing information on the birth abroad of a Filipino child. It is important for Filipinos permanently residing abroad to promptly register with the Consulate the birth of a child in order for such a birth to be registered with the Office of the Civil Registrar General in Manila.
When a child is born abroad to parents who are both Filipino citizens or to one (1) parent who is a Filipino citizen, i.e. one who has not been naturalized as a citizen of a foreign country, the child’s birth must be immediately reported to the Philippine Embassy or Consulate which exercises jurisdiction over the place of birth.
ONLY births that happened in the following places can be registered or reported at the Philippine Consulate General, Los Angeles, CA: (1) Southern California; (2) Southern Nevada (Clark, Lincoln and Nye Counties) and (3) Arizona;

Ideally, the birth must be reported to the Consulate within twelve (12) months after its occurrence. When the parents neglect to report the birth within twelve (12) months, the birth may nevertheless be recorded upon the determination of the consular officer of satisfactory evidence on the authenticity of the report. In these cases, the person who executed the report shall furnish the Consulate with an explanation surrounding the delay in reporting the birth.
Eligibility:
Born on or after 17 January 1973
- At least one parent indicated in the US birth certificate must have been a Filipino citizen at the time of the child’s birth
Born before 17 January 1973
- The father indicated in the US birth certificate must have been a Filipino citizen at the time of the child’s birth
Procedure and Requirements
NOTE: CIVIL REGISTRY REPORTING IS PRIMARILY DONE BY MAIL
- LETTER-REQUEST SIGNED BY THE INFORMANT. Please indicate your contact details (mobile no. and email address)
- FOUR (4) ACCOMPLISHED REPORT OF BIRTH FORMS (Click here for the form). Each form must be originally signed by the informant and duly notarized before a notary public.
- THE CHILD’S ORIGINAL BIRTH CERTIFICATE PLUS FOUR PHOTOCOPIES
- PARENTS’ ORIGINAL MARRIAGE CERTIFICATE PLUS FOUR PHOTOCOPIES.
IF MARRIED IN THE PHILIPPINES, marriage certificate must be issued by the Philippine Statistics Authority (PSA).
IF MARRIED OUTSIDE THE PHILIPPINES, marriage certificate must be issued by the civil registrar of the county/country where the marriage was contracted. For foreign marriage certificates issued in a language other than English, a certified English translation must be provided (the original translation plus four photocopies must be presented together with the original and four photocopies of the foreign marriage certificate).
- PARENTS’ BIRTH CERTIFICATES
IF BORN IN THE PHILIPPINES, birth certificate must be issued by the Philippine Statistics Authority (PSA).
IF BORN OUTSIDE THE PHILIPPINES, birth certificate must be issued by the civil registrar of the county/country where the birth occurred. For foreign birth certificates issued in a language other than English, a certified English translation must be provided (the original translation plus four photocopies must be presented together with the original and four photocopies of the foreign birth certificate).
- PARENTS’ VALID ID. Provide four photocopies
Examples: PH or US passport, PH national ID, state ID, driver’s license, among others
- PROOF OF FILIPINO PARENT’S/S’ CITIZENSHIP AT THE TIME OF CHILD’S BIRTH
Examples: Dual citizenship documents, US naturalization certificate, permanent resident card/green card, US visa (B1/B2 , K1, J1, etc.)
- FOUR PHOTOCOPIES OF THE DATAPAGE OF CHILD’S PASSPORT (if available)
- APPLICABLE FEE:
- $25.00 POSTAL MONEY ORDER/CASHIER’S CHECK PAYABLE TO “PHILIPPINE CONSULATE GENERAL” (preferably by mail through USPS or UPS)
- SELF-ADDRESSED AND STAMPED LEGAL-SIZE MAILING ENVELOPE (please specify priority mail with a tracking number)
ADDITIONAL REQUIREMENTS:
IF THE REPORT IS BEING MADE A YEAR OR MORE AFTER THE CHILD’S BIRTH, include a duly notarized affidavit explaining the delay in reporting the birth. Provide the original affidavit plus three photocopies ( Click here to download the Affidavit for Delayed Registration of Birth template )
IF THE PARENTS ARE NOT MARRIED, INCLUDE
- A duly notarized acknowledgement of paternity executed by the father. Provide the original acknowledgement plus three photocopies (Click here to download the Acknowledgment of Paternity template)
- A duly notarized affidavit executed by the mother allowing the child to use the father’s surname, if the child is using the father’s surname in the US birth certificate. Provide the original affidavit plus three photocopies ( Click here to download the Affidavit to Use the Surname of the Father template )
- Additional fee of $25 for the certificate of registration of the above documents (to be issued by the CONSULATE)
IF THE PARENTS WERE NOT MARRIED AT THE TIME OF THE CHILD’S BIRTH BUT SUBSEQUENTLY MARRY, INCLUDE
- PSA-issued certificate of no marriage record of the Filipino parent/s issued no earlier than three months prior to the birth report.
- A duly notarized joint affidavit of legitimation, to be submitted in person at the Consulate for notarization ( Click here to download the Joint Affidavit of Legitimation template )
- Notarization fee of 25$
