Refer to the core requirements for either ePassport renewal or new/first ePassport application.
1. If the minor is a dual citizen at birth (both Filipino and US citizen) born in the Philippines, present Consular Report of Birth, if applicable, and US passport plus a copy of each document.
2. If the minor is a dual citizen at birth (born to Filipino citizen parents in the US), a report of birth is required. Present the PSA copy of report of birth, or the local copy of the report of birth if registered less than a year from application of a new ePassport, and US passport plus a copy of each document.
3. Minor must be accompanied by either parent (if the child’s parents are married), by the mother (if the parents are not married), legal guardian or authorized adult companion.
4. The child’s PSA birth certificate or PSA report of birth is always required to show proof of filiation.
5. If the accompanying parent is not a Filipino citizen, the latter must present his/her foreign passport plus a copy of the data page and a copy of the Filipino spouse’s Philippine ePassport or other valid competent proof of identity.
6. If the minor is accompanied by an authorized adult, a Special Power of Attorney (SPA) executed by either of the minor’s parents (if married), by the mother (if the parents are not married), or the child’s legal guardian, as the case may be, to be presented with the adult companion’s passport or other competent proof of identity, and a copy of the passport or other competent proof of identity of the minor’s parents.
