A duly-accomplished visa application form (Form No. 2). This form may be accomplished by a parent on behalf of the applicant. For People’s Republic of China (P.R.O.C) passport holders, use this form)

Apostilized original and photocopy of applicant’s Birth Certificate issued by the County or State (this document would be necessary prior to boarding your flight to the Philippines and while going through Philippine Immigration).

Copy of data page of valid Philippine Passport of Filipino parent. If Filipino parent is a Dual Citizen, must also present original or certified true copy (with photocopies) of Dual Citizenship documents (these documents would be necessary prior to boarding your flight to the Philippines and while going through Philippine Immigration).

The foreign parent traveling with the minor child must also secure his/her own visa to enter the Philippines as a spouse of a Filipino national. For requirements, click here.

Original and Photocopy of both the Child’s Passport and Foreign Parent’s Passport.

Both Child’s and Foreign Parent’s passports must have a validity that must extend at least six (6) months beyond the length of proposed stay in the Philippines.

One 2”x2” colored photograph with white background of the applicant taken within the last six (6) months.

For foreign children with special needs, proof that Foreign Child is covered by Section 4 (a) to (d) of RA 7277.

Notarized Affidavit of Request, Consent and Guaranty executed by the Filipino parent. This Affidavit must be presented before immigration officials at the port of entry in the Philippines. The Philippine Bureau of Immigration will issue a Waiver of Exclusion Ground (W.E.G.) upon the minor’s arrival in the Philippines.

Proof of Parent/s financial capacity (e.g. latest print-out of credit card account(s), latest print-out of bank statements, ownership of real estate properties, and other documents showing the capacity to financially support said foreign child during his/her stay in the Philippines. You will need more than one type of proof of financial capacity.

Cashier’s check or US Postal Money Order pay to the order of the Philippine Consulate General Los Angeles in the amount of US$30.00 for the Visa Fee.

A photocopy of the applicant’s proof of round trip airline booking (unpaid reservation) to his/her country of origin or his/her next country of destination.

Letter request by the Filipino parent addressed to the Consulate General with copy of valid identification card issued by the Philippine government (i.e. Philippine passport). This letter must be NOTARIZED by a notary public in the Philippines.

Return envelope that is self-addressed with prepaid postage (PRIORITY MAIL with tracking number).

If in the Philippines and visa holder wants to extend his/her stay beyond 59 days, an application for extension of stay must be filed at the Bureau of Immigration in Manila, or the Immigration office nearest to the place where the applicant is temporarily residing. Four (4) 2″ x 2″ size photograph taken within the last six (6) months. The applicant’s signature must be affixed on the front side of the picture.

Medical Certificate (F.A. Form No.11) accomplished by a local physician. All supporting documents must be submitted in triplicate (three copies). A Chest x-ray plate in CD must accompany the certificate, together with the results for the following tests: 1) stool (ova/parasite and occult blood); 2) urinalysis; and 3) complete blood test (blood serology and chemistry).
