A travel document is allowed only for applicants who cannot be issued a Philippine passport and have an emergency travel to the Philippines such as due to medical or legal reasons or death in the family.
It is valid for 30 days from issuance and can only be used one-way to the Philippines. In order for you to leave the Philippines, you will need to apply for a new passport.

Print out/Copy of Valid Airline Ticket with departure date of less than one month from date of applying for a travel document;
If the airline ticket purchased for travel to the Philippines has a departure date of less than 48 hours from purchase date, please first email:

to alert the Consulate General on the urgency of your need. Using mail-in application in this case is not an option for lack of material time and in your email, you have to request for a walk-in application and attach your airline ticket as proof of the date of travel. -
4 pieces of 2x2 photos with white background taken at most six (6) months from date of applying for a travel document;

Request letter, stating urgency/need for travel, and indicating your phone number and email address for ease of contact. Include here, original or photocopy of Proof of Urgency (e.g, Medical Certificate, Death Certificate, Subpoena Ad Testificandum, etc.)

In case of lost passport, present the following:

Notarized Affidavit of Loss [click here for pdf ];

Birth Certificate and Marriage Certificate if married [click here if your Birth or Marriage was recorded by the Philippine Census/Philippine Statistics Authority]; and

Documents as proof of Philippine Citizenship (ex. Philippine driver license, baptismal certificate, voter's affidavit Professional Regulatory Commission Card, U.S Permanent Resident Card, etc.)

Cashier's check or Money Order pay to the order of the Philippine Consulate General Los Angeles in the amount of US$30.00

For Filipinos who acquired citizenship through naturalization or election: In addition to the requirements above, must also bring the following:

For naturalized Filipinos, Identification Certificate of Naturalization from the Bureau of Immigration;

For Filipinos who elected citizenship, Affidavit of Election of Philippine Citizenship and Identification Certification of Election from the Bureau of Immigration.

Note: Include in the mail packet, along with your application form and documentary requirements, a SELF-ADDRESSED STAMPED ENVELOPE with tracking number (use USPS Priority/Priority Express Mail Envelope. PLS. DO NOT USE FEDEX).
The Consular Officer reserves the right to require additional documents or to reject the Travel Document application pursuant to Philippine Passport Law and the Foreign Service Act. If your application is rejected, your documents will be returned to you using the self-addressed stamped envelope you provided.
Travel Document will be mailed back within 48 hours of receipt, provided all documentary requirements are in order.
