Requirements for the Issuance of a Provisional Certificate of Philippine Registry (PCPR)
- For Newly Built Vessels
- Special Power of Attorney notarized by a Philippine Public Notary issued by the importing Philippine Owner/Corporation stating his agent/representative who will file and claim the PCPR from the Philippine Consulate General LosΒ Angeles on behalf of the said Owner/Corporation.
- Completely filled up PCPR application, signed by the authorized company official/agent, including the photocopy of the authorized person’s valid passport. Download Form
- Valid Ferrying Permit issued by the National Telecommunications Commission of the Philippines
- Copy of signed, duly notarized and Apostilled Ship Building Contract between shipyard and owner
- Copy of Apostilled Builder’s Certificate
- Apostilled Certification from Bureau Veritas (BV)Β – Classification or Class Certificate and Survey Form
- Apostilled Bareboat Charter/MOA
- For Transfer of Ownership/Acquisition
- Special Power of Attorney notarized by a Philippine Public Notary issued by the importing Philippine Owner/Corporation stating his agent/representative who will file and claim the PCPR from the Philippine Consulate General LosΒ Angeles on behalf of the said Owner/Corporation.
- Apostilled Bill or Contract of Sale issued by the seller, signed by a duly constituted and authorized representative of the selling company
- Completely filled up PCPR application, signed by the authorized company official/agent, including the photocopy of the authorized person’s valid passport. Download Form
- Apostilled Certification from Bureau Veritas (BV)Β – Classification or Class Certificate and Survey Form
- Apostilled Ship Sanitation Certificate
- Apostilled Certificate of Ship’s Registry and Nationality
- Apostilled Ship Inspection Certificate by US Government authorities
- Latest and valid Apostilled Survey Report issued by a marine survey/class inspection organization accredited by the US Government and duly notarized by a Notary Public
- MARINA approval for temporary registration under Philippine flag of the vessel
- MARINA Endorsement to DFA
- DFA Communication to this Consulate for the issuance of the PCPR
- Valid Ferrying Permit issued by the National Telecommunications Commission of the Philippines
- Apostilled SOLAS Certificates
- Cargo Ship Safety Radio Certificate
- Cargo Ship Safety Construction Certificate
- Cargo Ship Safety Equipment Certificate
- Record of Construction and Equipment for Oil Tankers
