Passport Applicants who are Dual Citizens under RA 9225

REQUIREMENTS

If you are a dual citizen by birth (natural born Filipino born in the US ), then you are not covered by RA 9225. All you need to do is report your birth to the Philippine Consulate or Embassy that has jurisdiction over the place of birth before applying for a Philippine passport. For Adult Male Applicants

  - If Report of Marriage (ROM) was registered less than year from date of passport application: original and photocopy of the Report of Marriage (ROM); (For those under our jurisdiction, see Report of Marriage Requirements)
  - If ROM was registered one (1) year or more from date of passport application: bring original and photocopy of PSA ROM.
- In case of woman married in the Philippines or country other than the U.S.: original and photocopy of Marriage Contract duly authenticated by the PSA [apply here for your PSA Marriage Certificate]
For a married woman wanting to revert to her maiden name after Husband's death or legal dissolution of marriage: In addition to the applicable requirements in items 1-5, the applicant must bring:
- For widowed applicants - original and a photocopy of the husband's PSA death certificate or PSA issued marriage certificate with annotation of court decree of declaring husband's presumptive death. [apply here for your PSA Certificate]
- For annulled marriages - original and photocopy of annotated PSA Marriage Certificate with annotation indicating that the Philippine Court issued an annulment decree or decree of nullity.[apply here for your PSA Certificate]

For adopted adults: In addition to requirements above, the applicant must bring the following documents:
- original and photocopy of PSA Birth Certificate issued after adoption decree [APPLY HERE];

For legitimate minors: In addition to the requirements above, personal appearance of either parent is required during the application. If both parents are unable to appear, then the legal guardian must appear with the minor and bring the following documents:
- Affidavit of Support and Consent to Travel [click here for pdf ] executed by either parent indicating the name of child's travelling companion.
- Special Power of Attorney with an attached copy of either parent's valid passport authorizing a representative to assist the child in the application.
- Original and photocopy of the valid Identification card of the parent's representative.
- Photocopy of the valid PH passport or valid ID of either parent.
- Original and Photocopy of NSO/PSA Marriage Certificate of Parents
For illegitimate minors: In addition to the requirements iabove, personal appearance of the mother is required during the application. The mother must bring with the minor the following documents:
- Original and photocopy of her valid PH passport or valid ID.
For illegitimate minors acknowledged by the father: In addition to the requirements above, the mother must bring the minor and the following applicable document:
- If Report of Birth was registered less than one (1) year from date of passport application: the Report of Birth showing the child using father's surname.
- If the Report of Birth was registered one (1) year or more from date of passport application: original and photocopy of the PSA birth certificate reflecting the father's surname with the father's Affidavit of Acknowledgement and Consent to use his surname [APPLY HERE].
For minors legitimated by the subsequent marriage of parents: In addition to the requirements above, the either parent must bring the minor and the following applicable document:
- If Report of Birth was registered less than one (1) year from date of passport application: the Report of Birth showing the child using father's surname and annotation that child's status as legitimated.
- If the Report of Birth was registered one (1) year or more from date of passport application: original and photocopy of the annotated PSA birth certificate on the child's status as legitimated
- If born in the Philippines: original and photocopy of the PSA birth certificate with the annotation of the child's legitimated status.
- If both parents are unable to appear, then the legal guardian must appear with the minor and bring the following documents:

  2. Special Power of Attorney with an attached copy of either parent's valid passport authorizing a representative to assist the child in the application.

For adopted minors: In addition to the requirements in items 1 and 2, the adoptive parent/s must appear and bring the following documents:
- original and photocopy of PSA Birth Certificate issued after adoption decree [APPLY HERE];
- Certified true copy of the adoption decree issued by a Philippine court or foreign adoption decree recognized by a Philippine court;

OTHER INFORMATION

Processing Fee: $60.00 in cash, US Postal Money Order or cashier's check payable to the Consulate.
All passports are printed in the Philippines. The estimated time of your passport's arrival at the Consulate is six (6) to eight (8) weeks from date of processing.

For Releasing of Passports

- - Bring your own United States Postal Service (USPS) Priority Mail/Priority Mail Express Envelope. The USPS Priority Mail Envelope has to be self-addressed, postage prepaid and with USPS Tracking. To compute the amount of postage for a USPS Flat Rate Envelope, check here.
  - The Consulate shall not assume any responsibility for passports/documents mailed through USPS .
- For those whose passports were processed before 19 March 2020 and chose to pickup:
  - For adult applicant, please present:
    1. Your official receipt.
    2. If your old passport has not been cancelled, please bring and present the passport.
      - If the un-cancelled old passport was lost, bring and present PSA/NSO Birth Certificate, Marriage Certificate (if married) and a Notarized Affidavit of Loss

- - For representative of adult applicant, please present:
    1. Applicant's Official Receipt.
    2. Authorization letter if the authorized representative is immediate family member – spouse, father, mother, children who are at least 18 years old
    3. Notarized Special Power of Attorney if the authorized rep is not immediate family member - aunts, uncles, in-laws, grandparents, friends, neighbors, officemates, etc.
    4. Copy of valid ID of the passport applicant
    5. Copy of valid ID of the authorized representative
    6. If applicant's old passport for has not been cancelled, please bring and present the passport.
      - If the un-cancelled old passport was lost, bring and present applicant's PSA/NSO Birth Certificate, Marriage Certificate (if married) and a Notarized Affidavit of Loss

- - For minor applicant:
    - If applicant's mother will claim the passport, please present:

A Philippine passport remains at all times the property of the Philippine Government, the passport holder being a mere possessor. Hence, the loss, mutilation or destruction of the Philippine Passport constitutes an offense with penalties set by law.
